Scale 29 - 38,626 to Scale 34 - 43,693 with a professional bar at SCP 31 40,476**Hours Per Week: 36 hours Base Location: Tameside One Assessment Date: To be arrangedThe RoleWe re looking for 2 compassionate and skilled Transition Social Workers to support young people with autism, learning disabilities, and/or mental health needs as they move from children to adult services. You ll work closely with families and professionals across health, education, and care to ensure young people receive the right support at the right time.This is a unique opportunity to build meaningful relationships, advocate for young people, and shape care that promotes independence and positive outcomes.Key ResponsibilitiesDeliver person-centred social care by completing Care Act assessments, coordinating tailored support for individuals and carers, managing legal and transition-related documentation, and working collaboratively across services to ensure effective, needs-led care planning, delivery, and review.About You What We re Looking ForSocial Worker with experience, Qualified and registered with Social Work England.Experience working with young people with complex needs.Strong knowledge of the Care Act, safeguarding, SEND, EHCPs, and mental health legislation.Excellent communication, assessment, and coordination skills.A team player with a person-centred, strengths-based approach and able to work autonomously and as part of a team.Be part of a supportive, forward-thinking team that values collaboration, flexibility, and continuous learning.
