Practice Nurse Location: London Salary: £35,000 - £55,000 per year (Depending upon experience)Our client is a dynamic, multi-award-winning teaching Partnership, serving patients in the community for more than 30 years, in the Woodberry Wetlands region looking after circa 12,000 patients.The RoleThe post holder is responsible for the delivery of general practice nursing to the whole practice population. The focus of the role is both the delivery of evidence-based practice for patients presenting with a long-term condition, and the provision of preventative nursing activity to the practice population. As autonomous practitioners the nurse is responsible for the care they deliver, demonstrating critical thinking and skills in clinical decision making in the management of patients. They will work collaboratively within the practice team to meet the needs of patients, supporting the delivery of policy and procedures and providing nurse leadership as required.You will work within your scope of clinical practice and:
Assess, plan, develop, implement and evaluate treatment programmes that promote health and well-beingAssess, plan, implement and evaluate individual treatment plans for patients with a known long-term conditionProactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriateWork with other health care professionals to diagnose, monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needsReview medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocolsWork with patients in order to support adherence to prescribed treatmentsProvide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactionsPrioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency careSupport patients to adopt health promotion strategies that promote patients to live healthily, and apply principles of self-careSupport and manage health needs of women presenting for family planning and cervical cytology consultationsRecognise, assess and refer patients presenting with mental health needs in accordance with the national guidelinesImplement and participate in vaccination and immunisation programmes for both adults and childrenAdvise, support and administer vaccinations where appropriate for patients travelling abroadPromote and deliver evidence-based care for patients presenting with aural conditionsMeet the needs of patients presenting for opportunistic wound care
Skills and QualificationsEssential:
Qualified and registered NurseRelevant nursing/ health Degree in Primary CareRegistered with the Nursing and Midwifery CouncilMinimum of 1 year’s post registration experienceAt least 1 year’s recent experience of working in Primary CareExperience of nurse led management of long term clinical conditionsExperience of implementing protocols and clinical guidelines
Desirable:
Minimum of 2 years’ experience of working in a primary care environmentExperience in triageExperience in paediatric care
