In this role, you will:* Perform comprehensive patient assessments* Create and implement personalised care plans* Deliver evidence-based treatments to optimise patient outcomes* Collaborate with multidisciplinary teams to provide holistic care* Maintain accurate patient recordsYou'll work within the community setting, supporting a mixed caseload of adults and children, making experience in both highly desirable. A car driver is essential for this role due to travel requirements.Requirements & Qualifications:To be successful in this role, you will need:* A current and valid HCPC registration* Experience working with neurological conditions across both adult and paediatric caseloads* Strong communication and interpersonal skills* The ability to work independently and within a multidisciplinary team* A commitment to delivering high-quality patient care* A full UK driving licence and access to a vehicle (essential)This opportunity won't last long!
