Job Title: Community Physiotherapist (Band 6) Location: North West London Duration: Permanent Schedule: Monday to Friday, full-time Start Date: ASAP Rate of Pay: £30,000 – £50,000 per annum (depending on experience) Contact: Grace Snedden – What You’ll Be Doing: Join our team as a Community Physiotherapist and play a key role in supporting patients to regain independence and function within their own homes and community settings. In this role, you will:• Perform comprehensive assessments of patients with a wide range of conditions • Create and implement personalised care and rehabilitation plans • Deliver evidence-based treatments to optimise patient outcomes and independence • Coordinate care by liaising with families, carers, and wider multidisciplinary teams • Assess equipment needs to promote safety and function at home • Maintain accurate and timely patient records in line with professional standards • Promote health and wellbeing to prevent deterioration or hospital admission Requirements & Qualifications: To be successful in this role, you will need:✔ A current and valid HCPC registration ✔ Experience working with adults in community rehabilitation or acute settings ✔ Strong communication and interpersonal skills ✔ The ability to work independently and within a multidisciplinary team ✔ Commitment to delivering high-quality, person-centred care ✔ A full UK driving licence and access to a vehicle for community visits Working Hours & Contract Duration: This is a permanent full-time position, working Monday to Friday, with flexibility where needed. Salary ranges from £30,000 to £50,000 per annum, depending on experience.
